Lower Level Class Approval	obtain permission to register for class numbered less than 300, if it is not your 13^th credit.
Incomplete Grade Justification	contract between an instructor and student that documents the deliverables and timing for removal of an incomplete with a letter grade.
Masters Degree Advisory Committee	formalize the members of a Masters Degree (MS, MST & MEM) Advisory Committee. For full-time students this must be submitted before the close of your second semester of registration. Part-time and DE students must submit this form by the time they complete 12 credits or three semesters - which ever is earlier.
*Report of the Masters Degree Advisory Committee	document completion of requirements (course program, completion of thesis/indep. study) for the Masters Degree (MS, MST & MEM). Ph.D. Advisory Committee	formalize the members of an Ph.D. Advisory Committee. For full-time students this must be submitted before the close of your second semester of registration. Part-time and DE students must submit this form by the time they complete 12 credits.
*Report of the Ph.D. Advisory Committee	document completion of requirements (committee meetings, course program, examinations, advance to candidacy, completion of dissertation, submission of paper) for Ph.D. Degree. For full-time students the course program portion of this form must be submitted before the close of your second semester of registration. Part-time and DE students must submit their course program by the time they complete 12 credits.
Extension of Time Limit to Complete Degree	seek an extension beyond the set limits for completion of degree or support.
Approval for Summer Work Authorization	obtain approval to work on campus over summer without being registered.
On campus Employment Certification	This form must be completed for each international F1 student hired to work in a department and who therefore needs a Social Security number.
Transfer Credit	request transfer of up to 12 credits of qualified coursework taken at another university.
Dual Registration	simultaneous registration as an undergraduate student and a graduate student during the last semester of the undergraduate program or as part of a 5-year B.S./M.S. program.
Contract for readmission to regular graduate program	Contract between student and department outlining academic performance expectations readmission as a regular degree-seeking student following suspension. Required of suspended international students.
Survey of Post-Graduate Plans	obtain information related to your plans after graduation. This form must be completed and submitted to the Graduate Office no later that the last day to submit completion materials.
* Forms in progress reside in the file of each student in the Graduate Office. Students should deposit the form with the Graduate Office after each entry, and pick the form up for each new entry.
