RETURNING STUDENTS

   

 

At the beginning of each semester/summer session, print and fill out form below. Return the form to the Office of Counseling & Disability Services (OCDS) along with a copy of your class schedule. Depending on what your disability/accommodations might be, it would be in your best interest to request accommdation letters at least 2 weeks prior to the start of the semester. This will allow OCDS to better serve and accommodate the student in a timely fashion.

 

  • It is your responsibility to pick up and deliver your Accommodation letters to your professors as soon as possible, but no later then two (2) weeks of the start of the semester/ summer session.  If you have any questions please fill free to call our office at 575-835-6619.
